Every point you marked Nonconformity or Observation is listed here. Write the requirement, the evidence and the gap. Write the gap about the system, never about a person.
Grading rules
If your site has written grading rules, paste them here and they will print in the report. If you leave this blank the workbook’s definitions apply.
| Grade | The workbook’s definition |
|---|---|
| Major | The control is absent or has broken down, or the gap could directly affect product safety or legality |
| Minor | The control exists and generally works, and the gap is a lapse that does not by itself put product at risk |
| Observation | Not a nonconformity. The criterion is met and a weakness was seen that could become one |
Step 3: the report
The workbook assembles the rest. These four are yours to write.
